Passport Info

FSJMHA is voluntarily collecting all rostered volunteer and coaches vaccine passport information. This is being done to centralize and store all records for our rostered volunteers.  Disclosure of this private information will not occur, but in the event that a parent, other volunteers, or body of authority should question the validity or status of one of our valued volunteers, we would like to be able to say that all volunteers are vetted and proof of vaccination has been supplied - producing your passport will not be required again. 

You are not required to submit a copy of your passport.  This is purely voluntary.  

The aim of this initiative is to provide confidence to all parents and volunteers involved with FSJMHA that we are abiding by and are compliant with all PHO, BC Hockey and VIA sport mandates.  

Scanning of passports may still be required from time to time, or when you are acting as a spectator or travelling out of the association.
